*Payments may be made by cash, check, debit card or credit card. There is a
$25 charge for any returned checks.

MISSED APPOINTMENTS: Please notify me at least 48 hours in advance if you
are unable to keep your appointment. Failure to provide this notice of
cancellation, or failure to arrive for a scheduled appointment, will result in a
charge for the full session fee. Insurance will not pay for a missed
appointment.
At present, Dr. Hardebeck accepts the following insurance plans:

If your insurance company is
currently accepted:
If your insurance company is
not on the accepted list, or if
you do not have health
insurance:
  • Your insurance will be billed for
    service. You will be responsible for
    any co-payments as stated in your
    insurance plan. Co-payments are due
    at the time of service.*
  • You may be able to arrange with your
    insurance company a "single case
    agreement." This is sometimes done
    when no "preferred provider" is near
    your home; or if nearby preferred
    providers do not have any openings;
    or if no preferred provider offers the
    specialized service being sought. If a
    "single case agreement" is approved
    by your insurance company, they may
    pay full, in-network benefits. You
    would be responsible for any co-
    payments, which would be due at the
    time of service.*

  • You may pay for services out of
    pocket. Out of pocket payments are
    due at the time of service.* If you
    pay out of pocket, you will be given a
    receipt which you may submit to your
    insurance company; you may be able
    to recoup some of your expenses by
    submitting an "out of network" claim
    to your insurance company.

  • Out of pocket rates are $200 for the
    initial appointment, then $160 per
    hour for testing/assessments, or $140
    per hour for therapy.
